    Berlin Youth Wrestling reserves the right to publish your child’s individual picture/team picture in the local newspaper and on the Berlin Youth Sports Facebook page https://www.facebook.com/BerlinYouthWrestling/. By signing this form you agree to have your child participate in our club and agree to have your child picture to be used for club promotional purposes.
  • Each wrestler must have a signed parental consent and waiver in order to practice or wrestle. The club recommends that all children have a physical exam by his/her physician.

  • (1) The undersigned, on behalf of a minor child participant, understands and acknowledges that participation in the Berlin Youth Wrestling Club program may involve an element of risk of personal injury or property damage for all participants and may cause serious injury, or even death. The undersigned, on behalf of a minor child participant, and on behalf of the participant’s heirs, executors, administrators, and assigns, hearby releases, discharge, and agree to defend and hold harmless the Berlin Youth Wrestling Club, all Board of Director members, Coaches, Organizers, and Instructors from any and all injuries and property damages incurred by the participant and any and all claims of injury or property damage brought by third parties or other participants against the Berlin Youth Wrestling Club, all Board of Director members, Coaches, Organizers, and Instructors for the actions and/or activities of the participant during participation in the Berlin Youth Wrestling Club Program.

  • (1) As a Parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury.

    I have read the Parent Concussion and Head Injury Information and understand what a concussion is and how it may be caused.* I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected. I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me. I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach. I understand the possible consequences of my child returning to practice/play too soon.

  • Sportsmanship Agreement

  • Berlin Youth Wrestling Club was designed to help promote and develop the Wrestling programs for boys and girls in Kindergarten through Eighth grades.  The development will focus on providing the youth with the fundamental understanding and enjoyment of wrestling. Participation and sportsmanship will be stressed at all levels, with a primary concern for developing basic individual skills and general team concepts.

    Representatives of the Berlin Youth Wrestling Club including, but not limited to, board members, committees, and match officials have total control of, and complete authority over, all competition. All decisions made by these persons are final and have full support of the Berlin Youth Wrestling Club.

    These representatives are commissioned by the Berlin Youth Wrestling Club to take any action deemed necessary to guarantee a positive atmosphere for competition. To help ensure a positive atmosphere, athletes, coaches, and spectators are expected to do the following:

    • stand during the National Anthem
    • maintain self-control at all times
    • show respect for opponents
    • support your team in a positive manner
    • recognize and acknowledge good performance by all teams and individuals
    • be humble in victory and gracious in defeat

    Unsportsmanlike conduct, on the part of wrestlers, coaches, managers, or spectators will not be tolerated and may result without warning in expulsion (team and/or individual) from the Berlin Youth Wrestling Club.

    Unsportsmanlike conduct includes, but is not limited to, the following:

    • fighting,
    • “trash talking”,
    • taunting,
    •  foul language, and
    • verbal abuse/intimidation of opponents, game officials and/or representatives of the Berlin Youth Wrestling Club.

    No individual or team refunds will be given ifexpulsion occurs. It is expected that athletes, coaches, and spectators exercise totalcooperation with these representatives at all times.

    I have read, understand, and share the above philosophy, and hereby state that I will do my part in ensuring an enjoyable experience for all involved.
